    Job summary

    Alma Medical Centre provides excellent care to a population of approximately 11,600 patients. We are a training practice with 5 partners, salaried GPs, Advanced Nurse Practitioners and Primary Health Care support workers with a supportive administration and reception team. We are seeking to recruit a dynamic, organised and enthusiastic Practice Nurse to join our wonderful team.

    As a successful post holder you will work as part of a multidisciplinary primary health care team delivering primary health care nursing within your competencies. Responsible for ensuring the delivery of safe and effective general nursing care to the Practice population.

    Main duties of the job

    As a successful post holder you will work as part of a multidisciplinary primary health care team delivering primary health care nursing within your competencies. Responsible for ensuring the delivery of safe and effective general nursing care to the practice population.

    You will be responsible for the implementation of process of effective management of long term conditions, childhood immunisations and travel vaccination and screening programmes.

    The suitable candidate will be a registered nurse with experience working in General Practice desirably holding qualifications in asthma, COPD, diabetes, CHD, cervical screening.

    Pay rate will be subject to qualifications and experience.

    Job responsibilities

    Main Duties

  • Assist GPs to help deliver effective implementation of management of patients with chronic diseases as details by the Quality and Outcomes Framework, providing disease management clinics
  • Undertake wound care / management / dressing techniques
  • Provide an adult and childhood immunisation service working to Patient Group Directions
  • Health promotion, including breast awareness, cervical screening, and bowel screening
  • Cervical cytology
  • Deliver travel vaccination and health advice
  • Undertake male, female, and family planning assessments, give advice, and refer patients appropriately
  • ECGs
  • Offer guidance and support to healthcare assistants to improve standards of care, if needed
  • Support the practice in achieving quality indicators and targets of the GMS contract and any relevant National Service Framework
  • You will undertake a range of nursing assessments and provide appropriate care / treatment in conjunction with GPs, Nurse Practitioner, and Practice Nurses, or independently as appropriate for the need, according to practice policy, protocols, etc.. You will strive to improve standards of care and be involved in:

  • Contribute to the development of policies / protocols / guidelines within the Practice
  • Support the implementation of evidence-based guidelines in conjunction with the practice nursing team
  • Promote the continuing professional development of yourself and other nurses and HCAs within the Practice, working alongside healthcare assistants to help develop their skills
  • Provide professional and managerial support to healthcare assistants and review and monitor patients within Primary Care
  • Offer guidance and support to healthcare assistants to improve standards of care if needed
  • Undertake accredited training in specific chronic disease management as / if required, diabetes, respiratory, epilepsy, CHD, etc.
  • Contribute to the maintenance of the Practice disease registers
  • Provide advice on lifestyle issues, assist and support patients in changing their unhealthy behaviours, and refer appropriately
  • Recognise the signs of child abuse, family violence, vulnerable adults, substance abuse, addictive behaviours, suicidal tendencies, and be aware of local referral procedures
  • Ensure that current evidence based approaches to patients are standard practice
  • Apply infection control measures when collecting and handling laboratory specimens, disposing of waste materials, dealing with blood and body fluids spillages, and report and treat any sharps injuries
  • Undertake first aid and management of emergencies, burns, haemorrhage, hypo- / hyperglycaemic emergencies, asthma attacks
  • Undertake annual training and demonstrate competency in anaphylaxis and resuscitation techniques
  • Provide professional advice / support to healthcare professionals within the Primary Healthcare Team
  • Follow identified training needs, participate in training and facilitation of other Practice Nurses and Health Care Assistants
  • Participate in partnership programmes of awareness raising, detection, and prevention of chronic disease
  • Maintain a level of expertise in specific chronic disease management (when appropriate) and other pertinent areas of healthcare and seek to develop new and improved skills
  • Advise members of the public on the concepts of health promotion, education, prevention, and protection
  • Take responsibility for personal development and education
  • Actively participate in multidisciplinary education and training
  • Monitor and maintain consistency of professional standards and take appropriate action if standards are not met
  • Maintain professional development by attending relevant courses and study days to keep abreast of current knowledge and developments
  • Abide by the legal requirements and statutory rules relating to practice, and record and maintain periodic professional registration with RCN and NMC
  • Participate in an annual appraisal
  • Contribute to, and work effectively within, the Primary Care Team
  • Assist in clinical audit and the setting and monitoring of standards of care
  • Maintain accurate patient records
  • You are responsible for limiting your actions to those which you feel competent to undertake. If you have any doubts about your competence during the course of your duties, you should immediately speak to the Nurse Manager, in the first instance

    Registered Health Professional

    All employees of Alma Medical Centre who are required to register with a professional body, NMC, to enable them to practice within their profession, are required to comply with their code of conduct and requirements of their professional registration.

    Supervision

    Where the appropriate professional organisation details a requirement in relation to supervision, it is the responsibility of the post holder to ensure compliance with the requirement. If you are in any doubt about the existence of such a requirement, in the first instance advice should be sought from the Nurse Manager.

    Risk Management

    It is a standard element of the role and responsibility of all staff of Alma Medical Centre that they fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions, and the reporting of all incidents, near misses, and hazards.

    Records Management

    As an employee of Alma Medical Centre, you are legally responsible for all records that you gather, create or use as part of your work within the Organisation (including patient health, financial, personal, and administrative), whether paper based, or on a computer. All such records are considered public records, and you have a legal duty of confidence to service users. You should consult the Practice Manager if you have any doubt as to the correct management of records with which you work.

    Qualifications

    Essential

  • Registered General Nurse, Level 1
  • Evidence of Ongoing Post Basic Education at Degree Level
  • Competent in the Basic Nursing Duties Required for the Position
  • Desirable

  • Diploma or Degree Level Training in Diabetes, CHD, Asthma, COPD, and Contraception
  • Qualified in Cervical Cytology
  • Willingness to Undertake Further Training and Personal Development as Identified by the Practice
