The CLCH Planned Care Team in the Herts Division is really proud of our new model for triage and initial remote assessment for patients being referred to the community nursing team. We are looking for nurses who are confident in talking to patients, carers, and family members on the telephone or via video consultation. You will be making a difference by providing initial clinical advice and information, signposting to appropriate services, and initiating episodes of community nursing care. We have flexibility to employ full time and part time staff, with a minimum commitment of working 19 hours per week for substantive post holders.
