    An exciting opportunity has arisen for a full-time/ Part time Band 6Respiratory Nurse Specialist in the well-established Leeds CommunityRespiratory Team. The Leeds Community Respiratory Team is made up of SpecialistNurses, Physiotherapists, HOS-AR associate practitioners and clinical supportworkers providing care and treatment for patients with COPD and otherRespiratory disease, supporting admission avoidance, chronic diseasemanagement, pulmonary rehabilitation ,Home ward Respiratory and the home oxygenservice.

    Main duties of the job

    Youwill provide care to patients predominantly with Chronic Obstructive PulmonaryDisease (COPD) through different pathways, such as home visits or Clinics forhospital discharge as well as chronic disease management.

    The team run the Home Oxygen Service in Leeds which provides assessment andmonitoring for respiratory and non-respiratory patients requiring home oxygen.

    We are looking for passionate, dynamic and flexible individuals with someexperience of managing respiratory patients in a community or hospital setting.

    The individual is required to adapt to changing demands and workload tocontinue to deliver high quality care and demonstrate clinical decision makingworking independently or as part of the wider multi-disciplinary team.

    The respiratory service is a 7day service operating from 8:30am to 4:30pm, dueto the nature of the role, it is essential that you are able to travelindependently across the city and have access to a suitable vehicle forbusiness purposes.

    Job responsibilities

    The post holder will beresponsible for their own caseload within their speciality area as anautonomous practitioner. In doing so, they will assess, plan, deliver andevaluate patient centred programmes of care to a specialist level ensuring thatthey practice within their sphere of competence and knowledge. The post holderwill work to professional and regulatory body codes, standards and guidance atall times ensuring that their practice is grounded in evidence basedtheoretical and practical knowledge.

    Key Responsibilities

    1. Clinical

    Demonstrates a detailed theoretical andpractical knowledge of common aetiologies, pathologies, conditions andpresentations involving a critical understanding of theories and principles

    Demonstrates innovative and specialist skills toselect methods and tools to holistically assess, plan, implement and evaluatein a complex and specialised field of work.

    Demonstrates highly developed physical skillswhere dexterity and accuracy are important syringe pumps and infusions,manual treatments

    Makes consistent judgements requiring complex factanalysis, interpretation and comparison of options and ensures clinicalreasoning is reflected accurately in case records

    Engages and actively involves the individualand, if appropriate, their family/carer, in the assessment, planning,implementation and evaluation of programmes of treatment and/or care by using person-centredtechniques to promote a culture of involvement and empowerment

    Works to standards of proficiency identified bytheir professional and regulatory bodies performing clinicaltreatments/procedures to a competent standard

    Prioritises their own workload, and that of thewider team, within agreed objectives deciding when to refer to others asappropriate.

    Provides and receives complex, sensitive orcontentious information where motivational, persuasive, empathetic, negotiatingand reassurance skills are required whilst demonstrating an understanding ofbarriers to communication

    Approaches each individual with care, compassionand sensitivity ensuring that these values are reflected in all areas ofpractice including the management of any complaints and compliments.

    To comply with the organisations InfectionPrevention and Control requirements, including bare below the elbows dresscode for staff with a direct care delivery role

    Ensures that all objectives, for self and thosedirectly responsible for, are clearly defined within the wider Directorateframework and in line with Trusts objectives, using the appraisal process as avehicle for this.

  • Professional degree or diploma in relevant field
  • Post Graduate qualification in specialist field
  • Appropriate statutory professional body registration (NMC)
  • Must be willing to participate in any relevant training identified to develop skills required to carry out duties
  • Maintains a portfolio of CPD in line with regulatory body standards

  • Supporting Learning in Practice (SLIP) or equivalent Leadership and management training ILM 3 or equivalent
