    Job summary

    The homeless hospital discharge service provides enhanced inpatient care, and ensures safe, appropriate and sustainable hospital discharges that meet the needs of patients currently experiencing homelessness.

    The service works with a variety of hospital, statutory and voluntary Partners. The teams aims and objectives are to bring a wider influence to hospital, housing and social care systems to proactively identify and better respond to the needs of currently homeless and otherwise disenfranchised people.

    The Specialist Nurse works alongside the lead clinician for the service and a support worker, contributing clinical knowledge and expertise.

    As a key part of a multi-disciplinary intervention, the post holder assists patients identified within the organisation as experiencing homelessness to improve and maintain their health, by identifying and managing unmet health, housing and social care needs.

    Main duties of the job

    The post holder will:

    Be responsible for the identification of homeless/vulnerable patientswho attend A&E and/or are admitted as an inpatient.

    Be expected to carry out a holisticassessment in order to address physical health, mental health and social needs,taking into consideration the lifestyle, gender and cultural background andensure involvement with the patient, family, carers and significant others.

    Assess condition of patients including suitability for discharge.

    Work as part of the multidisciplinaryteam fostering good interdepartmental relationships and helping to achieve highmorale for patients and staff and continuous quality improvement.

    Promote health education involves patients and carers appropriately indecision making.

    Be involved in setting up support/carepackages based on their assessment including support with the patients housing,physical health needs, alcohol and drug dependencies, mental health issues andany other social care needs that the patient would benefit from been supportedwith.

    Provide direct support to the nursing team on the wards/departments toprovide a safe and timely discharge.

    Work closely with other members of the Homeless Discharge Team.

    Attend and contribute to multi-disciplinary meetings.

    Contribute to data collection and analysis.

    Be involved in community based work including involvement with theBRICSS intermediate care facility and street outreach as required.

    Job Title: Specialist Nurse - HomelessDischarge Service

    Job Purpose

    Key Roles and Responsibilities

    Participate in service development, identify and promoting the need forchange.

    Participate in teaching, mentoring and assessing of students, traineesand other professional staff.

    Partnership Responsibilities:

    To representthe service in a variety of contexts, deputising for the Lead Clinician asappropriate.

    To workcollaboratively with hospital, statutory and voluntary/third sector partners atall times in order to improve patient pathways.

    To developrelationships with partnership organisations as required.

    Tosupport and manage partnership staff within the team as necessary.

    Professional Responsibilities:

    To ensure mandatory training is up to date and proactively identify andaddress own learning needs relating to the role.

    To ensuring nursing registration is up to date, revalidation requirementsare met and maintain professional standards.

    Manage safeguardingconcerns appropriate to your role and level of training.

    Person Specification

    Qualifications

    Essential

  • Registered Nurse on the NMC register (Adult, Mental Health or Learning Disability field).
  • Degree in nursing or a related subject or equivalent level of study or relevant experience.
  • Good knowledge of the social determinants of heath and how this directly affects patients.
  • Desirable

  • Masters degree in relevant subject.
  • Mentorship qualification.
  • Leadership training.
  • Specialist knowledge regarding the effective management of patients with personality disorder/complex trauma.
  • Knowledge regarding rights to housing and benefits, housing options and local authority processes.
  • Knowledge regarding the support options available for people experiencing homelessness in local communities.
  • Knowledge on immigration status and its impact on welfare rights.

  • Minimum of 1 year post-registration nursing experience.
  • Experience of managing patients with mental capacity and/or safeguarding issues.
  • Desirable

  • Experience within inclusion health.
  • Experience of setting up, and maintaining relationships with a wide variety of relevant external partners.
