    Clinical Nurse Specialist - Wakefield, United Kingdom - The Mid Yorkshire Teaching NHS Trust

    Job summary

    The post holder will be responsible for themanagement of a defined caseload of patients / service users and will highlyprovide specialist nursing support, education, and care to that specific group.They will ensure continuing responsibility for the assessment of care needs,the development, implementation, and evaluation of programmes of care,including setting of standards of care and to take a lead role in establishingevidenced based practice.

    This will be the first Clinical Nurse Specialist post in the area of arrhythmia, but the post holder will join a large cardiology department that already has an established CNS team caring for patients with other cardiac diseases. Close support will be provided by a consultant cardiologist especially in the early stages of the role.

    As this is a new role within our department, the post holder will need to work flexibly as it develops. There is significant scope for development and adaptation of the role over time, in order to best meet the needs of our patients. The post holder will lead this development.

    The post holder will be dynamic, enthusiastic, and act as a role model for colleagues. They will be expected to develop and maintain up-to-date clinical knowledge and competence and will be given appropriate opportunity for study.

    Main duties of the job

  • The post holder will work as part of a multi-disciplinary team providing clinical care to patients with arrhythmia.
  • The post holder will be primarily based at Pinderfields Hospital, within the cardiology department.
  • The core clinical aspect of the post will be specialist outpatient arrhythmia clinics, working closely with a consultant cardiologist with a specialist interest in heart rhythm management. This will involve:
  • Clinical assessment of patients
  • Requesting and interpreting relevant investigations
  • Independently prescribing drugs relevant for the treatment of arrhythmia and monitoring anti-arrhythmic drug therapy where required. This will also include counselling for and initiating anticoagulation when required.
  • Assessment, counselling, and shared decision making with patients regarding onward referral for electrophysiological procedures in tertiary care.
  • Triage and advice provision for primary care arrhythmia referrals via the Trusts Shared Referral Pathway (eConsultation)
  • Audit and governance of the arrhythmia service through various means ( waiting times, patient reported outcome measures)
  • Participation in the development of local arrhythmia guidelines

    Key Responsibilities

  • Responsibility for a defined caseload of patients and will provide an effective and high-quality patient-focused service that is sensitive to the needs of users and providers.
  • Organises, prioritises, and evaluates workload based on accepted referral criteria and discharge criteria in line with operational protocols.
  • Will provide clinical, social and psychological support to their patient group using advanced communication skills, education and health promotion to enable them to reach an optimum level of independence and health.
  • Will have responsibility for the delivery of specialist nursing skills within complex pathways and will be able to provide specialist advice and support to a defined caseload and to other health care providers.
  • Responsible for providing evidence-based practice.
  • Will have responsibility for leading identified and agreed aspects of the service, in the development of care systems, protocols and guidelines working as part of a multi-disciplinary team and developing excellent interdepartmental relationships with excellent communication with a focus on quality improvement.
  • Will ensure cost effective use of all resources and appropriate governance management.
  • Lead and promote programmes of health education and health promotion relevant to service.
  • Work within national policy frameworks relevant to the service and trust local guidance.
  • Review, interpret and act upon diagnostic investigations and point of care testing which may affect, change to individual care plans, liaising with appropriate medical staff where necessary
  • Deal effectively and professionally with patient / service user issues collaborating with PALS as appropriate and take a proactive approach to resolving issues and learning from complaints.
  • To maintain accurate appropriate and concise patient records within agreed databases and electronic systems
  • Contribute and lead the multi-professional approach in the management of patients / carers within the specialist area in relation to ongoing care needs.
  • Act as a resource for specialist knowledge for clinical staff on wards / departments
  • Ensure effective multi- disciplinary communication.
  • Provide mentorship and preceptorship as required to all staff in the team or students on placement.
  • Produce an annual report for the service and board reports / other reports as required.
  • Ensure you work within an agreed divisional work plan.
  • Responsible for managing staffing capacity and service need for effective clinical cover.
