    Band 5 to 6 development role


    The Berkshire Early Intervention in Psychosis Service (EIP) is looking for a passionate and enthusiastic band 5/6 mental health practitioner to join their innovative and dynamic team.

    We are a specialist service working with individuals experiencing a first episode of psychosis or who are presenting with an at-risk mental state, between the ages of 18 to 65.

    We operate a Monday to Friday 9am to 5pm service, although some flexibility is required.

    To act as care coordinator, in accordance with the Care Programme Approach (CPA) policy under the supervision of the Clinical Lead.

    To undertake comprehensive assessment of health and social needs together with the client and their family to implement a plan of care and provision of NICE recommended interventions.

    To work closely with other stakeholders to ensure the client has a comprehensive and holistic care plan to meet their well being needs.

    To monitor and assess the physical health of service users in line with Trust policy and national guidelines.
    Plan and provide treatment in collaboration with a multi disciplinary team and ensure clients are on the correct care pathway

    Diploma or degree level in mental health nursing, and registered with NMC

    ~ Experience or being a care coordinator.

    Education/Qualifications/Training
    Previous knowledge and experience of working in a care environment and with service users who have complex mental health needs

    Competent IT skills and the ability to navigate around various systems and software packages (such as Outlook, databases, MS office and the internet)
    Able to travel and undertake home visits as required by the job role
