    Central London Community Healthcare Trust have an exciting opportunity for a Senior Community Nurse within the Brent Health Matters Clinical Service .

    Main duties of the job The post holder will work as a complex case manager, without direct supervision. You will be involved in delivering integration, working with GP's, Social services and the third sector organisations.

    You will be providing case management, clinical staff support, mentoring, clinical assessments for people who have difficulty accessing healthcare services and with some of the social determinants of health inequalities for the most vulnerable communities, supporting the service at GP practices, community outreach events and where appropriate, supporting healthcare closer to home.

    The service currently operates from hrs, Monday to Friday however, weekend and evening working would be required, dependent on the service needsThe post holder will be a Registered Nurse, with at least 3 years community experience of managing a complex caseload of patients. You will be responsible for a caseload of patients with complex health and social care needs. You will work autonomously to case manage these clients and use advanced nursing assessment skills, to manage their conditions in the community.

    • Working closely within the multidisciplinary team structure, the post-holder will be required to provide support to service users who experience severe and enduring mental health problems.
    • You will manage a caseload and take responsibility for undertaking initial and comprehensive assessments of health & social care needs, independently or with colleagues, the day-to-day coordination of the multidisciplinary teams mental health clinical activities, ensuring that clinical priorities are achieved, maintained and aligned with the highest standards of care.

    • Registered Nurse with current UK NMC registration
    • Degree/Diploma in adult nursing
    • Evidence of post registration education / leadership courses including appraisals
    Desirable
    • Advanced Nurse Practitioner qualification
    • Mentorship, student supervisor qualification
    Knowledge and Experience Essential
    • Experience of working and delivering healthcare to patients with complex needs or hard to serve service users in the community
    • Evidence of theoretical knowledge and practical experience in a wide variety of clinical areas within secondary and primary care
    • Experience assessing, planning and implementing care plans
    • Experience of providing evidence-based care in the community
    • Recent evidence of liaising and negotiating with system partners to facilitate quality outcomes for patients.
    • Experience of organising and managing team caseload
    • Experience of teaching and mentoring junior staff and undertaking appraisals.
    • Knowledge and experience working within Health Inequalities.
    • Experience managing long term conditions in the community.
    • Partnership working with multi agency/system partners to deliver proactive effective seamless services in the community.
    • Evidence of Research Based Practice and service innovation
    • Flexible approach to change and service innovation
    • Recent examples of service innovation and change management experience
    • Experience of using a range of Clinical / IT systems
    • Experience of working with MDT and System partners across organisational boundaries
    • Working knowledge of National policies and procedures across health and social care and its implementation or application

    • Working knowledge of Health and social care policies and procedures and application within the community.
    • Knowledge and understanding Clinical Governance and evidenced based care.
    • Knowledge and experience of clinical audits, research, and service innovations
    • Good Communication Skills and I.T skills
