    Overview

    London Borough of Harrow is looking for a Care Manager in the Integrated Neighbourhood Teams to provide support to people and their carers ensuring that a strengths-based approach is used to empower the person/carer and connect them to their community. You must identify the needs of people and carers and give choice and control over the delivery of services whilst promoting independence, in accordance with current legislation, and with the policy and practice of the local authority.

    The post holder is expected to work with all agencies in a multi-disciplinary way, to ensure the best co-ordination and efficient delivery of services to people. As change is a continuing feature of the organisation, a flexible approach is needed to carry out duties. Direct contact with people who may be distressed, have specific needs including, physical, sensory, or learning difficulties, so it is imperative that staff show sensitivity, tact, and discretion always.

    You will be expected to work across the geographical area of Harrow therefore a full UK driving licence and use of a vehicle for work purposes is desirable, but not essential.

    About You

    The successful candidate will have:

    • Knowledge of a range of interventions and services provided by Social Services and other agencies including the private and voluntary sector.
    • Knowledge and understanding discriminated against groups and the ability to ensure that services are appropriately delivered to meet their needs.
    • Ability to demonstrate structure of own time and to prioritise competing demands to ensure all essential work is completed.
    • Ability to produce and use workload information and acquire skills to use IT systems are available.
    • Good verbal and written communication skills.
    • Experience of working with people with a physical, learning, or sensory disability and their carers.
    • Willingness and ability to undertake duties outside of conventional office hours as and when necessary.
