This role is for YOU.
You will be the point of contact for all Inpatient referrals, responsible for co-ordinating patient flow, from admission to discharge within the IPU. You will work within a multidisciplinary team fostering positive relationships with our Therapies and Hospice at Home teams. You will be a main point of support for patients, their families, carers and colleagues.
You will be confident in taking the lead at multidisciplinary team meetings, engaging in discharge discussions and holding the medical team accountable for their role in projecting discharge dates. Outside of the organisation, you will foster close relationships with key referers and promote the Hospice to help widen our reach to more patients.
The successful applicant must be flexible and available to work all shifts across 7 days. Experience of working in palliative medicine is desirable but not essential. An enhanced DBS check is also required.

The role willinvolve fostering close relationships with key referrers and also promoting thehospice to new referrers so that we can widen our reach to more patients with apalliative care diagnosis.
Supportedby the wider multidisciplinary team, the post holder will need to workcollaboratively with members across the organisation to optimise our onehospice approach, and fully optimise our internal services to effectivelypromote onward care in the community
Specific Objectives

KeyTasks and ResponsibilitiesTo meet the above broad objectives, the post holderwill have responsibilities in four main areas of work as follows:

1) To lead the co-ordinating of the dailyoperational requirements of discharge priorities.
2) Central point of contact for referralsand discharges.
3) To liaise with patient and family tosupport with preferred place of care and death.
4) To lead and be the responsible personfor all discharges within the IPU.
5) To be the main co-ordinator with outsideagencys ( in supporting patient flow.
6) To be an integral part of MDT
7) To foster positive internalrelationships with the wider MDT who will be able to promote dischargeopportunity; Therapies and wellbeing, hospice at home
8) To lead daily bed meetings to supportwith flow of patients through the building
9) Raise and process relevant and timelyreferrals as and when required to assist discharge planning.
10) As the central point of contact fordischarge planning provide regular discharge updates to patients, familymembers, ward teams, internal and external support services/agencies.
11) Arrange the details of any follow-upcare.
12) To represent the Hospice and championhigh-quality care at the end of life and collaborative working across the widersystem working towards extending our reach across the community
13) Maintain and monitor themes in patientadmission and discharge data; to evidence wide and fair spread of access toservices & recognise opportunities for further service development andexpansion.
Educational
1)Drivea positive culture of discharge focus, presence in MDT meetings is essential,and the post holder will be tasked with creating daily focus on dischargediscussion, holding the medical team accountable for identifying a patient as beingoptimised for discharge, or projecting a expected discharge date to deliver to.
2)To ensure all staffcomplete Mandatory training as set by KHH.
3)To attend anytraining or development required by the hospice
4)Supportthe nursing team to confidently and proactively engage in early dischargediscussions, maintaining a strong ethos of discharge being a collaborativefocus.
5)Activelyengage in known referral partners, to promote collaborative working, andrecognising a positive professional overlap in providing joint care to improvepatient outcomes for our shared patients.
6)Toparticipate in the Annual Performance Review
Clerical
1) Lead patient flow within the IPU
2) Be the main contact within the IPU,taking telephone calls from relatives and professionals
3) Participate in administration tasks asdirected by registered nurses

1) Take all measures to ensure the safetyand welfare of all patients and staff, having due regard to the Health andSafety at Work Act
