Description
As a community nurse, you will be responsible for delivering high-quality nursing care to individuals and families within their homes and community settings.
You will work collaboratively with other healthcare professionals to assess, plan, implement, and evaluate care plans tailored to meet the unique needs of each patient.
Your role will involve providing clinical interventions, health education, and support to promote independence, well-being, and self-management.nKey Responsibilities:
Conduct comprehensive assessments of patients' physical, emotional, and social needs in their home environment.nDevelop individualized care plans in collaboration with patients, families, and multidisciplinary teams.nProvide skilled nursing care including wound care, medication administration, and symptom management.nDeliver health education and support to patients and their families on managing their conditions, promoting health, and preventing complications.nCoordinate care and liaise with other healthcare professionals, community services, and social care agencies to ensure continuity and integration of services.nAdvocate for patients' rights and preferences, respecting their autonomy and dignity at all times.nMaintain accurate and up-to-date documentation of assessments, care plans, and interventions using electronic health records systems.nParticipate in ongoing professional development activities to enhance clinical skills, knowledge, and competence.nAdhere to professional standards, regulatory requirements, and organizational policies and procedures.nContribute to service development, quality improvement initiatives, and research projects as appropriate.
Qualifications and Requirements:
Registered Nurse (RN) qualification with current registration with the Nursing and Midwifery Council (NMC).nExperience working as a registered nurse, preferably with experience in community or home-based care.nExcellent clinical assessment and communication skills.nAbility to work autonomously and collaboratively within a multidisciplinary team.nCompassionate, patient-centered approach to care delivery.nKnowledge of relevant legislation, policies, and best practices in community nursing.nValid UK driving license and access to a vehicle (for roles requiring home visits).nCommitment to continuous professional development and lifelong learning.
