    Job Description - Support Coordinator
    Permanent, Part Time (14.Plus, Brilliant Benefits including Health Cash Plan
    We work with adult male customers who have been homeless and have a variety of support needs. These include offending histories, substance misuse, mental health issues and learning disabilities. They either live at the supported housing service on Rosary Road or at one of our move on properties.
    Typical day as a Supported Housing Co-ordinator

    Creating support plans with your customers and coordinating our small team of Support Workers working to the plans .

    Helping customers with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation if they are able.

    Carrying out risk assessments, support planning, goal setting and regular reviews.

    Experience of creating person-centred support plans and supporting colleagues in working to work to the plans.

    A highly organised and practical approach, to deal with housing management queries and are confident to work with our contractors.

    Experience in safeguarding, risk assessments and confidentiality, working collaboratively with your colleagues across the organisation, and external agencies.

    You'll join our manager Bex who's just completed her first year at Home Group but has years of experience in the housing sector and is super passionate about what we do and her team and is a real people person too, a self-described 'sociable creature' who enjoys a good pot of coffee.

    We're a really supportive bunch and often enjoying 'team time' in form of a local lunch, knowledge check session or meal out.

    We work with our customers providing 24-hour support, which means you'll be working on a rota basis. You will take your turn working days, evenings, weekends, and bank holidays.
    Where we're flexible with you, we do need to meet the needs of our customers, but we aim to give you the time you need to spend time doing the things that matter to you
    Able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues.
