    Job Title: Macmillan Palliative and End of Life Care (PEOLC)Transformation Lead

    Accountable to: Jane Wheeler, NWL ICB Programme Director

    Responsible for: Supporting NWL implementation of the newmodel of care for adult community based specialist palliative care across allproviders

    Location: all provider bases and the 15 Marylebone Roadoffice of the NWL ICB

    Main duties of the job

    The North West London Palliativeand End of Life Care (PEOLC) Transformation Lead hosted by Royal TrinityHospice will be primarily responsible for supporting the implementation of thenew adult community specialist palliative care model across the ICS.

    Youwill bring together statutory and non-statutory partners, including thecharitable hospice sector, across health and care to embed local priorities tobring about quality improvement in community based specialist palliative carefor the local population.

    The post holder will support the statutory coreresponsibility of the Integrated Care System (ICS) to ensure that system wideaccessible high quality, safe palliative, and end of life care, tailored to theneeds of the individual (Health and Care act 2022) is available.

    Main duties

  • Lead on the implementation of the new model of carefor adult (18+) community-based specialist palliative care services across NWL,addressing the eight key issues and developing the five enablers transformationalwork for successful delivery.
  • Facilitate coordination and collaboration amonghealthcare stakeholders for an integrated approach to delivering the enablerswhich underpin the new model of care addressing inequalities, commonapproaches to training, digital and information management, and workforcedevelopment
  • Develop a programmatic approach to the workrequired across NWL to deliver on the changes agreed in the new model of careand ensure clear ownership and timelines for change.

    Operational

    Tooversee the implementation of the new model of care, engage and liaise with keystakeholders, in particular:

  • To supportthe strategic direction and implementation of the new model of care
  • To supportthe identification and sharing of best practice#
  • To operate ina highly political and sensitive environment
  • Support theportfolio of initiatives in demonstrating value for money for the current spendthrough tracking, managing, and delivering agreed benefits.

  • Implementationof the new model of care for community based specialist palliative care reflectPEOLC national and local priorities
  • Lead andco-ordinate the delivery of project plans, allocating tasks as appropriate,identifying risks, issues, and dependencies, considering best practice andcurrent options, and ultimately making decisions in the best interest of theproject
  • Pro-activelymanage stakeholders, respond to, and resolve conflict between differentstakeholders when this arises through facilitation or other appropriatemechanisms
  • Produce ahigh standard of work supporting the delivery of projects on time and in acost-effective manner
  • Maintain theproject initiation document and associated plans with regular team meetings tomonitor progress and resources
  • Ensure theflexibility of the project if required to meet conflicting/changingrequirements.
  • Responsiblefor the planning and organisation of numerous events/meetings. Ensuringcommunication tools are used to their maximum value for circulating theminutes, agenda, and presentations in a timely manner

  • Ensure thatthe projects maintain business focus, have clear authority and that thecontext, including risks, are actively managed in alignment with the strategicpriorities of the PEOLC programme
  • Financial and Physical Resources

  • Act in a waythat is compliant with Standing Orders and Standing Financial Instructions inthe discharge of budget management responsibilities where appropriate
  • Responsiblefor ensuring adherence to the budget and ongoing monitoring of expenditureagainst budget and ensuring the appropriate documentation is available forscrutiny where appropriate
  • Responsiblefor making recommendations, providing advice and able to prepare strategicreports/briefings for the Head of Department, Steering/Reference Groups andothers as required

    Qualifications

    Essential

  • Educated to Masters level or equivalent level of experience working at a senior level, supported by a project management qualification for example PRINCE2, Agile or Managing Successful Programmes (MSP).

  • Extensive knowledge of specialist areas, acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or experience.
  • Desirable

  • Registered healthcare professional with relevant professional registration (such as nurse, occupational therapist etc.) or a manager with relevant healthcare experience.

  • Experience of cross organisational or system wide transformation.
  • Substantial experience of working in the PEOLC specialism.
  • Experience of leading, managing and delivering significant change projects involving stakeholders.
