    NHS Specialist Nurse - Birmingham, United Kingdom - University Hospitals Birmingham NHS Foundation Trust

    Description

    To lead the coordination of discharge arrangements for patient with complex needs as part of the National Hospital Discharge service, ensuring that the patient is at the centre of all discharge planning.

    In partnership with members of the multidisciplinary team the post holder will develop, implement and evaluate seamless specialist service, ensuring patients receive the highest standard of clinical care.

    To lead on the discharge planning and coordination for patients within the Trust who have complex healthcare needs.
    To provide effective liaison between health and social care professionals and other relevant agencies to support discharge.
    To update and maintain data bases.

    In the event this is not possible, the team must ensure the patient is placed with services relevant to their needs, that opportunities of independence and self-management are maximised and that reliance on long term care is mitigated wherever possible thus reducing the likelihood for readmission.

    Registered Nurse on the NMC register.

    • Maintained relevant continuing professional development in the past 3 years which supports revalidation.
    Experience in an acute hospital/community setting.
