- Work in partnership with voluntary and statutory sector organisations ( Age UK South Gloucestershire, Age UK Bristol, Alliance Homes, British Red Cross, NHS, and South Gloucestershire Council) to design and deliver a new approach to non-regulated adult social care and community-based support to speed up discharge from hospital and/or avoid readmission.

Job summary
Southern Brooks is looking for a new Community Social Care Manager to manage a team of workers who support adults being discharged from hospital or living at home with no social care package, and work towards preventing readmission.
Our Hospital Link Workers have been working within Southmead Hospital for the past two years, establishing a service that helps patients get out of hospital and back into their own homes, helping them to access support to enable them to live independently.
The Welcome Home project started at the end of 2023, and picks up where our Hospital Link Work ends. Working in partnership with Age UK South Gloucestershire, the team offers support at home for those who are ready to be discharged from hospital or who have recently been discharged. Our service can help people settle back home and rebuild their confidence for living independently at home.
To apply, you will need a experience working in a management role for 2 years in a related setting and role (for example, social care, care settings, therapeutic environment), proven experience of using a person-centred approach with service users and proven understanding of mental health and barriers to accessing services.
As a vital member of our team and in return for your passion, empathy, and commitment to leading, motivating and supporting the team, we will provide a favourable salary of £30,255 pro-rata, excellent annual leave and sickness benefits, and will even give you the day off for your birthday
Main duties of the job
Working in collaboration with our Head of Health & Wellbeing, you will oversee both teams, directly line managing both the Welcome Home Team Leader & Hospital Link Worker Team Leader, bringing both teams together to promote working together as one cohesive service.
