    CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting

    The South and West Hertfordshire Health Care Partnership (SWHHCP) are striving to reduce health inequalities and improve the health and wellbeing of its residents with a key focus on prevention and are delivering a number of key transformation project in order to implement an ambitious plan, as set out by the Hertfordshire and West Essex Integrated Care System.

    We are extremely proud that the first Covid-19 virtual hospital in the country is in our portfolio and the success of this service has inspired the system. Heart Failure and COPD Virtual Hospitals are now operational and soon to launch are the Acute Respiratory Illness, Frailty and Diabetes pathways.

    In order to further drive forward the success of the virtual wards we are recruiting a strategic and forward-thinking individual to oversee the delivery and performance of our virtual ward portfolio on behalf of the SWHHCP.

    Main duties of the job Although this role is hosted by Central London Community Hospital Trust (CLCH), the successful candidate will work in partnership with clinicians and operational managers from West Herts Teaching Hospital Trust (WHTH), CLCH and system partners to ensure effective utilisation and delivery of all the virtual hospital pathways. The Performance and Delivery Lead will have an expanding portfolio, taking ownership and leading on Virtual Ward development, prevention of admission and supported discharge initiatives under the SWHHCP transformation programme.

    To date, the wards have delivered significant bed day savings through reduced length of stay (LOS), reduced admissions and have improved prevention of admission.     • To provide strategic leadership to the Virtual Hospital in all current pathways,, together with the future development of the model, together with additional initiatives as agreed by SWHHCP
    • The post holder will demonstrate understanding of the national strategic direction of the VH model,, ensuring th VH performance and delivery operates in line with relevant guidance, policies, and procedures and maintaining the SWHHCP VH programs position nationally as an exemplar for VH care
    • To work collaboratively with partners in the SWHHCP to ensure effective and efficient delivery of all pathways within the healthcare system
    • To be responsible for the delivery of monthly performance reporting for all pathways and ensure standard operating models for performance management are deployed.

    • Experience in working with senior colleagues in supporting significant delivery of performance management.
    • Demonstrable track record of performance management
    • Experience in working with other agencies & a multi-disciplinary team.

    • Experience of working across a range of NHS settings e.g., acute, commissioning, community
    Skills Essential
    • Highly developed analytical skills, being able to interpret diverse and complex information spanning multiple organisations and use judgement to draw conclusions. Also, the ability to interpret, compare and present such information to different audiences.
    • Proven strong communication skills, credibility at senior level and with external stakeholders. Particularly in building and managing interpersonal and team relationships with strong and effective listening skills
    • Sound knowledge and practical experience of working with complex projects centered on data and/or clinical systems
    • Sound knowledge of performance management practices
    • Demonstrate political judgement and astuteness in understanding and working with complex policy and diverse interest groups.
    • Knowledge and understanding of NHS and public sector organisational structures and roles
    • Highly developed leadership and Influencing skills with the ability to enthuse, motivate and involve individuals
    • Ability to be intellectually flexible and to look beyond existing structures, ways of working, boundaries and organisations to produce more effective and innovative service delivery and partnerships
    • Evidence of excellent, written, verbal and presentation communication skills, effectively relaying and receiving complex information.
    • Ability for frequent concentration for analysis, report writing, meetings etc. with frequent interruptions to deal with service issues
    • Strong numeric skills and accuracy
    • Keyboard skills, using Microsoft Excel, Work and PowerPoint to design, create and input to complex spreadsheets and reports
    • Problem solving skills and ability to work through to successful resolution with the ability to prioritise workload
    • Ability to effectively plan, prioritise, organise, monitor and control all activities within areas of responsibility to meet deadlines and operational requirements in most effective and best value way
    • Evidence of personal contribution as a strong team player, facilitating effective team working to deliver outcomes
