Job summary
The Multiple Disadvantage Team has an exciting opportunity for a dynamic, flexible and highly skilled STR worker. The team work with clients who present with complex co-occurring needs, including, but not limited to, mental health, substance & alcohol use, homelessness, offending and trauma backgrounds. The role includes providing care for a caseload of clients supporting longer term assessments to understand the complex interplay of co-occurring needs. Supporting the stabilisation of clients in order to better understand their needs and allow them to engage with services in a way to address these needs. Clients may also be under the care of existing services but require short term scaffolding support to prevent service users falling out of services.
Excellent networking and communication skills are essential with the role involving a great deal of multi-agency working. The role requires a great deal of innovation and adaptable communication and engagement skills in order to provide meaningful outcomes to this client group.
A successful candidate will be able to demonstrate a proven track record of working with clients who experience multiple disadvantage and of working in a joined up multi-agency way.
Main duties of the job
Responsible for providing the link into the care coordination process for an allocated number of individual service users. This will include attending and actively participating in training sessions, team/care plan review meetings and supervision as appropriate.
To ensure that service users engage effectively with the agreed Care Plan and access appropriate services provided on a regular and consistent basis.
Positively promote independent living of service users within their community.
Developing a rapport based upon companionship and friendship but within appropriate and transparent boundaries.
Provide regular and practical support to service users and their carers in developing and managing dignity and independence.
Provide support with daily living; "living ordinary lives"
Help service users gain access to resources to include benefits and welfare rights.
