    Job summary

    Thisrole will operate within the Frailty Team which forms part of our wider LivingWell Team made up of Frailty nurses, Social Prescribing Link Workers, Healthand Wellbeing Coach and Care Coordinators. This is a new role for our network,expanding our Frailty Team to address the needs of our population.

You will be deliveringpatient care for our frail and housebound patients throughout the organisationunder the guidance of our Frailty Matron.

    About us

    North and South Gloucester Primary Care Network (Brockworth Surgery, Churchdown Surgery, Hucclecote Surgery, Longlevens Surgery and The Alney Practice) cares for 58,000 patients on the eastern side of Gloucester. The PCN offers services across all five practices including pharmacy, first contact physios and our extensive living well team.

    Job responsibilities

    TheFrailty HCA and Wellbeing Coordinator will play an important role within ourPCN working closely with our GP practice teams, our PCN Living Well Team andwider health and social care and community colleagues. You will be deliveringpatient care for our frail and housebound patients throughout the organisationunder the guidance of our Frailty Matron.

    The key responsibilities of the roleare outlined below:

    ProvideHCA patient care to our frail and housebound patients in their home includingbut not limited to blood pressure checks, venepuncture, undertaking INRs,simple dressings, blood sugar testing and dementia reviews.

    Assessfor and order equipment for patients.

    Bringtogether a persons identified care and support needs and explore their optionsto meet these into a single personalised care and support plan. Identifyingwhere additional input is needed from a frailty nurse or the wider primary careteam.

    Ensure that people have goodquality information to help them make choices about their care and structureconversations using a coaching approach.

    Improve continuity of care byacting as a point of contact for people, families and professionals.

    Leadthe management of the frailty email inbox, ensuring that all messages are dealtwith efficiently and any necessary actions are assigned.

    Wherenecessary coordinator and run MDT meetings.

    AttendMDT meetings and carry out resulting actions agreed during the meeting.

    Providecoordination and navigation for people and their carers across health and careservices, alongside working closely with our Living Well Team (SocialPrescribing Link Workers, Frailty Nurses and Care Coordinators) and all membersof our primary care teams.

    Workclosely with our Care Coordinators to provide timely access to healthcare athome for those approaching end of life.

    With the support of the Practice and Network proactively identify acohort of people in need of anticipatory coordinated support, using localknowledge and population health intelligence.

    Qualifications

    Essential

  • Good standard of education with 5 C/GCSEs or equivalent
  • Desirable

  • Care Certificate
  • Training in health coaching/motivational interviewing or equivalent
  • Experience

    Essential

  • Experience of working in the NHS (preferably in Primary Care) as an HCA or other clinical role.
