    Support Network - Extra Care

    Spinnaker House, Rock Ferry, Wirral is our 5th purpose built extra care scheme to be delivered by June 2024. The supporting local authority has commissioned an experienced registered care providers to deliver care and support 24/7 to enable people aged 55+ to remain independent and living well. The facility will have a dedicated Torus housing team who will work collaboratively with the care provider to ensure all residents are able to access opportunities to actively engaged in social activities and events on their schemes and in their local communities. The scheme includes commercially operated bistro and salon which will offer an exciting opportunity for a local business to deliver services to residents and people in the surrounding areas.

    This is an important role in which you will:

    • Assist and empower residents to maintain independent living to enable people to manage their homes and tenancies safely.
    • Record the risks and needs of residents and agree interventions that will enable people to remain at home using a case management approach.
    • Make appropriate referrals and work collaboratively with other agencies to ensure residents are receiving the care and support they need to enable them to remain at home.

    To be successful in this role you will:

    • Have experience of working with older and or vulnerable people within a housing or social care environment
    • Understand the social, physical and mental health needs of an ageing population and the ability to promote their general wellbeing and quality of life.
    • Have a track record of completing assessments and developing support plans with targets and outcomes for clients.
    • Have experience of managing a case load and multi-agency working.
    • Have experience of crisis management, risk minimisation and of making decisions in difficult situations
    • Manage your personal resilience when dealing with difficult situations and identifying mechanisms to deal with these.
    • Have knowledge and experience of Safeguarding Practices and Procedures
    • Be committed to ED&I in the workplace and in the delivery of services to customers in the scheme.
    • Use a range of ICT software packages and systems
